The cover artwork was painted by Rodney Matthews, the famous English Illustrator in fantasy style, created covers for works by many other artists (Scorpions, Thin Lizzy and others) the Title is borrowed from the novel by Alexander McArthur and Kingsley long's "No Mean City" (1935). With this album the band's sound became heavier, given the addition of guitarist Zal Cleminson of the Sensational Alex Harvey Band. The popularity of the album allowed the group to come up with a few big names such as Thin Lizzy, in their round of 1978/79.

The creation of this album was inspired by Cooper's stay in new York hospital, where he was treated for alcoholism. To work on the album, Alice invited three former band members Elton John: poet Bernie Taupin, guitarist Davey Johnstone and the bass player Dee Murray.
